Code 38 Error. Window Driver USBSTOR.sys


My Win 8 x64 desktop suddenly stopped recognizing USB storage devices (Thumb Drives & 2 External HDD). When working through Device manager, all show yellow exclamation marks and working through properties, all say "Windows cannot load the device driver for this hardware because a previous instance of the device driver is still in memory. (Code 38)". The driver in question is USBSTOR.exe. I think this is a generic windows Driver, so Win 8 is likely broken in this regard. I've rebooted many times, removed the devices using the control panel and swapped USB ports with no fix. The driver rollback option is not available & Control Panel says the driver is up to date. Not sure where to turn. Would like to avoid Reset and Refresh due to the time demands of setting up programs again. May try System Restore if no direct fix works. I'm having this problem with my flash disks on W8 Pro with Media Center x64:

"Windows cannot load the device driver for this hardware because a previous instance of the device driver is still in memory. (Code 38)

The driver could not be loaded because a previous version of the driver is still in memory."

After OS installation, the flash disks are still detected and can be seen on "My Computer." But after around 10 times of sticking in/out the flash disk, it can no longer be accessed. And the specific disk drive has the above error code displayed on Device Manager.

Any ideas guys?

BTW, I'm using HP Probook 4445s notebook and the flash disk/s is/are working fine on other notebooks/pc. I also had the latest BIOS update for my notebook. I also did safely disk removal when the drive is still functioning normally.

Hi! My father's computer been broken and he thought it was a virus attack. The problem is that it won't boot even though I tried all the choices (safe mode, boot from last good configuration,etc.). None of them worked. It kept repeating and repeating.

It's an upgraded version from Windows XP (to Windows 7).
Error code: 0xc0000039
Missing driver: win32/drivers/joyar.sys

What are the reasons that it happen?
How do I fix it?
Is only the solution to fix it is through its installation disk(repair) or by reformat?




i have been having bsod problems since i installed windows 7 on my computer. every where i post says that its a different issue. it really has its moments sometimes it goes weeks and months without a bosd and then all then its a every day or multiple times a day. the past 2 days its been acting up again and i have 3 memory dump files and it looks like ntoskrnl.exe is casing 2 of the crashes and dxgmms1.sys caused the other one. i have ran the debugging tools and bluescreen view i will show bluescreen viewer first.

im only showing the file that was highlighted red. i think that is the only one that matters. (let me know if im wrong.)

Anyway, I did a fresh install for him when I gave him the computer, basically just wiped everything. It has been working fine for a few months and then all of a sudden he started getting hard drive errors when starting up the computer. I ran quite a few programs such as Seagate, WD Utility programs as well as PassMark Disk Checkup and they all said the reallocated sector count has failed. I included this information in the zip file under DriveInfo.txt. I know this is the wrong area for this issue but thought it might be relevant.

I did as much research as I could and realized I would need to replace the hard drive. We cannot afford this right now so he is continuing using the computer with the knowledge we may lose everything on it at some point.

However, now he is getting the BSOD with a Window Driver Framework violation error. I am not sure if they are related or not. He has started playing Skyrim on here and while it worked fine I thought I would update his graphics driver (nVidia) from their website. It has been working great but started this message about a week afterward. Again, not sure if these things are related or not.

He uses Avast antivirus.

The computer gets pretty hot too. I have taken apart the computer and checked the fan and cleaned it in the past, but it doesn't seem to help all that much.

Attached is all of the info requested except RAMMon. I tried to open the program and I keep getting a "Unable to load DirectIo driver" error.

edited to add one more thing: The mouse on here stops working too. A general restart will usually fix it. Otherwise it works with a wireless mouse. I have done everything to try and fix this too, gave up and bought the wireless mouse. Maybe it has to do with the hard drive failing too... not sure.

At shutdown an unsigned driver (strln.sys by Veritas Software Inc) is
doing things that cause data to be corrupted. I caught it by using
"verifier.exe" which checks drivers in WinXP. It really likes to eat
my Mozilla profiles for some reason. But I'm scared it may be doing
other things as well. So far, using scandisk hasn't found any errors
on the hard disk (Thank god).

There must be some way to tell what software added this driver so I
can remove it! How do I tell what software on my machine has added
this unsigned driver? Unsigned drivers don't come with WinXP do they?
Is there a safe way to not load this driver (and still have XP boot?)

I have no scheduled tasks and I use no backup type software, which
seems to be what Veritas Software makes (http://www.veritas.com). I
also turned off Start-Up programs and disabled one service (Turned off
virus software. The rest are all Microsoft and one Nvidia graphics
card service.)

I have a HP pavilion dv9917cl laptop. Nvidia Geforce 7150M/630M
Integrated ethernet adapter, Nforce 630M.

Updated to Windows 7 32-bit from Vista 32-bit, custom (start from scratch) install.

Initially my ethernet adapter showed up in Device Manager as Nforce Networking Controller, not working properly with a code 10 error. Windows update provided a chipset download, where I manually picked out and installed the ethernet adapter driver (Browse for driver -> let me manual pick -> Have disk -> )

So now in Device Manager my ethernet adapter shows up fine, wired LAN connection shows up in Network Connections etc.

EXCEPT, now no matter what I always get "No Network Access" or "No Internet Access" no matter where I plug in my ethernet cable. Of further interest is this: my IP, DNS, DHCP information all shows up under "details" from the LAN wired connection properties, but My physical address is 00-00-00-00-00-00.

Wireless is working 100% okay. I need to be able to use wired, however, for a number of reasons.

I nabbed drivers off the HP website, the Vista 32-bit drivers for the nvidia chipset, but there was no change.

My biggest question: is this a driver problem? or Windows 7?

You have three connector types there. The only router console I have
ever logged into was accessed by connecting a DB-25 to the console and
connecting an RJ-45 to my laptop. I then logged in through HyperTerminal.

For your router console, are you saying the RJ-45 goes into the router
console and then you are trying to connect the DB-9 converted to USB to
your computer? Have you tried it the other way around? Also, have you
tried it without the DB-9 to USB adapter?

Sorry I'm not much help. Maybe someone here has their CCNA and wouldn't
mind chiming in

Hi All
I've been suffering from random BSOD's for a good while now (before I OC'd it, before you say anything :P) but they've recently gotten more and more common (1-3 in an evening), and it's getting a teensy bit annoying :P
It started getting worse after I went from Vista to 7/XP in August, and I rearranged the RAID at the time so I doubt there was any leftover files to screw anything up. Not long after, the RAID rearranged itself after a BSOD, going from one 250Gb volume and a 1.5Tb volume to 2 790Gb volumes. Both OS's boot and run fine, (apart from the BSODs lol), but the larger volume is inaccessible. I don't know if it makes any difference, but since the volume went I've been using my USB external hard drive for storage.
The BSODs are usually error code 50 or 01, but sometimes there is another one (I can't remember the code) with usbuhci.sys mentioned. They happen irrespective of what I'm doing so far as I can tell, but mostly occur after it's been on for a while.
I've attached everything the SF Diagnostic tool chucked out and screens of CPUz as well, and I think I've covered my system specs in my profile fairly comprehensively, sorry if I missed anything. Your RAM settings are wrong, load bios with the optimized defaults and if it doesn't help then set the timings manually 9-9-9-24.
Also check the RAM sticks manual -.pdf or its package with the specs - to verify the timings, frequency, and the voltage.

Hi,
I already read some threads about my problems, but I thought every BSOD depends on the Computer you use. So, I often have the BSOD, ntoskrnl.exe + 70740 or nvlddmkm.sys, when I am playing games like COD 7, but it also happens randomly or when I start the OS.

This is usually caused by drivers using improper addresses

1. Reinstall NVIDIA driver (you may want to try a different version) with the use of the driver sweeper:

My system get freezes everything and suddenly goes into BSOD stating Driver Power State Failure. I am attaching the mini dump along with the screen shots.
With Bluescreen View i could find out that it caused by ntoskrnl.exe

Usual causes: Device driver.

Your latest dump file lists usbhub.sys as the probable cause. This is a Windows System file and for it to be the cause of your crash is highly unlikely.
Old and incompatible drivers can and do cause issues with Windows 7, often giving false error codes. Random stop codes can often indicate hardware issues.

As a Priority:

Norton is known to be a cause of BSOD's on many Windows 7 systems. I suggest that you uninstall Norton using the Norton Removal Tool in Safe Mode. Download Blue Screen friendly Microsoft Security Essentials as its replacement. Make sure Windows firewall is enabled!
